Please bring the following items with you to your appointment

  • Your insurance card or other health coverage information. We will verify your insurance plan at least once per year and will scan your cards into our EMR system. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt. We accept a variety of insurance plans. Please check the insurance tab to see if your insurance plan is on our list. Please give our office a call to discuss further as recently many insurance plans have various new networks that require referrals.
  • A list of all your medications and dosages

Medical Records

If in the future you need to request certain medical records from our office, please be aware that the request could take up to five business days before you receive the information. We will make every effort to fulfill all medical records requests as quickly as possible.

Billing Information

We accept Visa, MasterCard, Discover, American Express, Care Credit, Cash and Check. Payment is due when services rendered.

Surgery

If you require surgery, our physicians will thoroughly explain the operative procedure and the recovery process . Our staff will pre-certify your procedure and discuss any financial implications with you.

Facilities

Our cardiologists operate out of Medical Center of McKinney, Medical Center of Plano and Heart Hospital Baylor Plano.

After Hours Process

We always have a physician on call in case of after hour emergencies. If you need to contact a physician after hours, call the regular office number and our answering service will notify the physician on call to contact you.

Refills

Routine prescription refills will be filled between the hours of 8:00 AM and 5:00 PM Monday through Friday. Please have your pharmacy send us a request.

Scheduling Policy

Except in the case of emergencies, it is our policy to see only patients with an appointment. We fill our schedule well in advance with, in some cases, people who have waited weeks for their appointment time.

Please remember that we have reserved appointment times especially for you. Therefore, we request at least a 48 hour notice in order to reschedule your appointment. This will enable us to offer your cancelled time to other patients.

If you are unable to keep your scheduled appointment time, please call our office at least 48 hours in advance in order to avoid a missed appointment fee. This charge is not covered by insurance. Your phone call is critical in helping us provide continuous care to all of our valued patients.

Missed appointments for Office Visit Appointments will incur a $50.00 charge.

Missed appointments for Nuclear Stress Tests will incur a $250.00 wasted dosage fee. Medications are ordered for these exams that are specific to your height and weight and cannot be saved or used on another patient.

For VEIN PROCEDURES such as Endovenous RFA, Sclerotherapy or Phlebectomy, failure to provide such notice will result in a charge of $150.00.
